This checklist is provided for your use in identifying all of the information and documents needed for the new group insurance business that you wish to place with Standard Insurance Company. To ensure an accurate evaluation and allow us to promptly issue a contract for the groups that are approved, please submit these items no later than three days after the effective date of the group policy or plan document.
