A Variety of Plans

Optima CareSM Dental Plan: Traditional indemnity plan ideally suited for an employer that seeks maximum plan flexibility with variable claim allowances, deductibles, coinsurance choices and plan maximums.

Banner Dental CareSM Plan: Provides employers with a comfortable, lower cost alternative to Optima Care Dental. It includes the most frequently used dental benefits that employers can enhance with additional options.

Balanced CareSM Dental Plan: A good choice for an employer that wants to balance cost and quality care by covering essential procedures to maintain oral health while controlling expenses through procedure limitations.

Voluntary Dental CareSM Plan: Voluntary coverage for an employer that wants to offer group dental coverage to employees without much, if any, cost to the employer.

Preferred CareSM Dental Plan: Participating Provider Organization (PPO) plan for an employer that wants a choice of flexible plan designs that provide employees with an incentive for using participating dental providers.

High/Low Optima CareSM Dental Plan: A plan for an employer that wishes to balance cost concerns with a choice of coverage levels that meet the financial needs of individual employees.

The Standard SelectSM Group Dental Plan: Available to small businesses with two or more eligible employees, this coverage is designed to meet the specific needs of smaller companies.
