We’re no longer providing or requiring two of our forms to be given to the client at the time of application; form 7127, the Minnesota Insurance Guaranty Association notice, and the accompanying form 4421, the Minnesota Insurance Guaranty Association notice delivery receipt. Form 7127 is issued with the contract.
