Updated Minnesota Disclosure Forms Now Available

We've updated our disclosure forms in the application packet for Minnesota to comply with a new state statutory requirement. All fully underwritten applications will now require a notice regarding limitations for mental health or substance use disorders. This form must be signed and returned by applicants in Minnesota.

All documents and packets are up to date in our Applications and Forms directory and in eApp. Any packets or documents downloaded and saved before today’s newsletter should be deleted, as they are obsolete.

If any outdated forms are received after Dec. 10, we will work with you to ensure the appropriate disclosure is signed and returned by your client.
