Basic Life and AD&D Coverage
The City of Tampa provides a basic amount of Life and AD&D insurance at no cost to you if you are an eligible active employee. Your Basic Life and AD&D coverage becomes effective on the first day of the calendar month that follows or coincides with 90 days as a member.

Your Basic Life coverage includes:
- Travel Assistance: Offers aid before and during your trip, including visa information, health inoculation recommendations, credit card and passport replacement and connection to medical care providers
- Life Services Toolkit: Provides resources for financial planning, estate planning assistance, health and wellness, identity theft prevention and funeral arrangements

You may elect a basic amount of Life insurance if you are an eligible retired employee. You must apply and agree to pay premium before your coverage becomes effective.

- Coverage Highlights
- Former employees in the following categories who retired on or after October 1, 1988: All elected, appointed and unclassified officials; City Attorneys; Classified and Unclassified full-time Managers; Police personnel at the rank of Captain or above; full-time Supervisors and Professional, Administrative and Technical Employees
- Employees under the Police Benevolent Association and Police Lieutenants who retired on or after October 1, 2009
- Employees under the International Association of Firefighters who retired on or after October 1, 1989
- Employees under the Amalgamated Transit Union who retired on or after October 1, 1993

If you are an active employee, you have the option to increase your coverage at your employer's competitive group rates.

Enrollment
Active Employees: To enroll in Additional Life insurance, please complete the enrollment form within 31 days of your date of hire or within 31 days of your Qualified Status Change (QSC). You may also enroll during your annual enrollment period.
Retired Employees: To enroll in Basic Life insurance, please complete the enrollment form within 31 days of your retirement date.
