Your agency has access to the Electronic Medical Questionnaire through your My Home account with
The Standard. If your agency already uses eApp, the questionnaire is the application supplement in the same format and platform.

As an agency, you can start the Electronic Medical Questionnaire on behalf of your producers. You only need some basic client information to get started. In three simple steps, your agency can send the questionnaire to your client for completion and e-signature. No agency or producer signature is required for the Electronic Medical Questionnaire.

PRODUCER submits request to the agency to launch the Electronic Medical Questionnaire.

Provides applicant’s first name, last name, last four of SSN, state of residence, email address and cell phone number.

AGENCY STAFF launches the Electronic Medical Questionnaire, including the client info provided. Next, sends questionnaire link by email or text to the client.Client completes the Electronic Medical Questionnaire and signs.Completed and signed questionnaire submitted to The Standard home office.

If any of your producers use eApp, they also have access to the Electronic Medical Questionnaire. Ask your sales consultant if you’d like to set up producers to use eApp and the Electronic Medical Questionnaire.
